In this on-site role, you will perform comprehensive nursing assessments, develop and manage individualized care plans, and collaborate closely with physicians, social workers, and the interdisciplinary team to ensure safe transitions of care. You will support discharge planning, connect patients with community resources, and provide culturally responsive education to patients and families.
